Missed Intradural Lumbar Disc Herniation

Abstract

Intradural lumbar disc herniation is rare. The diagnosis of intradural disc herniation can be made, with difficulty, using several neuroradiological modalities. In a 63-year-old man, intradural lumbar disc herniation occurred with severe dural adhesion. We missed intradural disc herniation during the first surgery. We performed the inevitable second surgery with a durotomy. Intraoperative vigilance and detection is of the utmost importance, because an intradural disc is usually not recognized from preoperative magnetic resonance imaging or computed tomography scans. A high index of suspicion is necessary for the diagnosis of intradural disc herniation, especially when severe dural adhesions are found and the intraoperative findings are different from the preoperative radiological images.
